The Population Health Management & Integrated Care Pathways Training Course is a practical, management-focused program that prepares health system leaders to move from managing separate episodes of care to managing the health of a defined population. The course covers population segmentation, risk stratification, disease registries, integrated care pathway design across primary care, hospital and home care, and the governance needed to run population health programs at scale. Health systems organized into clusters or provider networks need reliable ways to understand their population, identify high-risk and rising-risk groups, and coordinate services so people move smoothly between settings. Participants learn to combine health record, claims and registry data and turn the results into targeted programs and coordination arrangements. The program is particularly relevant to ministries of health, health clusters, payers and provider networks implementing new models of care, where progress often stalls because of unclear guidance, fragmented data and stakeholder resistance. The course addresses these barriers through practical governance, financing and change management tools. Learning combines case studies, data exercises and pathway mapping workshops. The focus is on organizing and managing care, not clinical practice, and participants leave with a draft population health management plan.
