Course Overview

The Strategic Health Purchasing & Provider Payment Training Course is built for the purchaser side of universal health coverage: national health insurance agencies, social health funds and ministries that decide which services are funded, from which providers and on what payment terms. Rather than running an insurance company or a hospital billing office, participants learn to use pooled funds actively to improve access, quality, efficiency and financial protection. The course examines purchasing governance and the purchaser-provider split, the design and costing of explicit benefit packages, priority setting with health technology assessment, and the full range of provider payment mechanisms, including line-item and global budgets, fee schedules, capitation, per diem and case-based payment using diagnosis-related groups. Particular emphasis is placed on the incentives each payment method creates, blended payment models, performance-based payment, selective contracting and empanelment, contract monitoring, and the information systems a purchaser needs to price services, adjust for risk and detect unintended provider behavior. The programme also addresses purchaser accountability to government, members and the public, reporting on financial protection and service coverage, stakeholder engagement and the sequencing of payment reform. Through case studies and design exercises, participants build a practical strategic purchasing roadmap for their own health financing system.

Learning Objectives

  • By the end of this training course, participants will be able to:
  • Explain the role of strategic purchasing within universal health coverage.
  • Distinguish active strategic purchasing from passive budget allocation and claims reimbursement.

  • Prepare provider contracts with clear service, quality, reporting and payment terms.
  • Establish purchaser accountability, monitoring and public reporting arrangements.
  • Develop a phased provider payment reform and strategic purchasing roadmap.

Who Should Attend

This course is designed for senior officials and managers in national health insurance agencies and health funds, including heads of purchasing, benefit package and provider payment teams, contracting and provider network managers, actuaries, health economists and health financing specialists. It is also suitable for ministry of health and ministry of finance officials responsible for health budgets, health insurance regulators, policy and planning directors, and the finance and contracting managers of large public and private provider networks that negotiate with purchasers. The programme is particularly relevant to organizations implementing or expanding mandatory health insurance and universal health coverage schemes, as well as development partners and advisers supporting health financing reform.

Learning Outcomes

  • Upon completion of the course, participants will be able to:
  • Map how revenue collection, pooling and purchasing interact in their health system.
  • Assess current purchasing practices against strategic purchasing principles.
  • Define the purchaser's legal mandate, decision rights and relationship with providers.
  • Draft benefit package inclusion and exclusion criteria with transparent revision rules.
  • Use costing and budget impact estimates to test the affordability of coverage choices.
