Course Overview

Root Cause Analysis (RCA) is most effective when investigations are structured, evidence-based, and led by professionals who can guide teams toward identifying underlying causes rather than focusing on symptoms or assigning blame. Effective facilitation and investigation leadership are essential for understanding complex incidents, operational failures, quality deviations, safety events, and recurring performance problems. This course develops the practical capabilities required to lead RCA investigations, facilitate multidisciplinary teams, manage investigative discussions, evaluate evidence, challenge assumptions, and distinguish root causes from contributing factors. Participants explore investigation planning, team roles, interviewing techniques, analytical tools, conflict management, and the development of corrective and preventive actions. Through realistic scenarios, facilitated exercises, and investigation simulations, participants strengthen their ability to lead objective investigations, maintain team accountability, document defensible findings, and translate conclusions into measurable improvements. The course is applicable across industrial operations, engineering, maintenance, quality management, healthcare, financial services, and other environments where systematic problem investigation is essential.

Learning Objectives

  • Establish a structured approach to planning and leading Root Cause Analysis investigations.
  • Define investigation scope, objectives, responsibilities, and decision-making authority.
  • Facilitate multidisciplinary investigation teams toward evidence-based conclusions.
  • Apply questioning, interviewing, and active listening techniques to obtain reliable information.
  • Distinguish root causes, contributing factors, symptoms, and immediate triggers.
  • Manage disagreements, competing interpretations, and cognitive biases during investigations.
  • Select and apply appropriate RCA tools based on the nature and complexity of an incident.
  • Evaluate evidence quality, test hypotheses, and validate investigative findings.
  • Develop corrective and preventive actions linked directly to identified causes.
  • Strengthen investigation reporting, stakeholder communication, and follow-up accountability.

Who Should Attend

RCA team leaders, investigation coordinators, and incident investigators. Quality assurance and quality control managers and specialists. HSE professionals, safety officers, and incident investigation personnel. Engineering, maintenance, reliability, and operations managers. Continuous improvement, operational excellence, and process improvement professionals. Risk management, compliance, and governance specialists. Production supervisors, department heads, and technical team leaders. Project managers and professionals responsible for investigating operational failures. Internal auditors and professionals involved in corrective action management. Professionals expected to facilitate cross-functional problem-solving and improvement teams.

Learning Outcomes

  • Lead investigation meetings using structured facilitation methods.
  • Establish clear team responsibilities and maintain productive multidisciplinary participation.
  • Conduct effective interviews and gather relevant, verifiable evidence.
  • Build incident timelines and organize facts into a coherent sequence of events.
  • Apply RCA techniques such as 5 Whys, Fishbone Diagram, Fault Tree Analysis, and causal factor analysis.
  • Identify and address assumptions, biases, incomplete evidence, and conflicting accounts.
  • Evaluate proposed causes and verify whether they adequately explain the event.
  • Formulate corrective and preventive actions that address verified root causes.
  • Present investigation findings clearly and monitor action implementation and effectiveness.
