Course Overview

This advanced training course provides a practical framework for strengthening patient safety and clinical risk management across hospitals and healthcare organisations. It focuses on moving beyond reactive incident management toward proactive identification, assessment, and control of risks before they result in patient harm. The programme examines clinical risk identification, risk assessment and prioritisation, adverse events, near misses, incident investigation, root cause analysis, and corrective and preventive actions. It also connects clinical risk management with clinical governance, healthcare quality, regulatory compliance, and continuity of care. Particular attention is given to high-risk clinical processes, including medication safety, patient identification, clinical communication, care transitions, surgical and procedural safety, falls, infection prevention, clinical documentation, and medical equipment. The course also addresses the role of leadership, communication, and organisational culture in creating safer healthcare environments. Through case studies, incident analysis, risk assessment exercises, and investigation simulations, participants develop practical capabilities to build clinical risk registers, analyse contributing factors, establish safety indicators, monitor corrective actions, and convert lessons learned into sustainable improvements in patient care.

Learning Objectives

  • Develop an integrated framework for patient safety and clinical risk management.
  • Identify clinical risks and assess their likelihood, severity, and potential impact.
  • Apply proactive methods to identify risks before they develop into patient safety incidents.
  • Analyse adverse events, near misses, and serious safety events systematically.
  • Apply root cause analysis to identify human, organisational, and system-related factors.
  • Develop corrective and preventive actions that address underlying causes.
  • Strengthen safety practices across high-risk clinical processes.
  • Improve incident and near-miss reporting systems and organisational learning.
  • Establish and maintain clinical risk registers and prioritise critical risks.
  • Develop patient safety indicators and monitor recurring risks and performance trends.
  • Strengthen leadership, governance, and safety culture in clinical risk management.
  • Develop practical plans for reducing clinical risks and sustaining patient safety improvements.
