Course Overview

The Operational Loss Event & Risk Incident Management Training Course equips operational risk professionals, risk managers, internal control and compliance officers, and business managers with the knowledge and practical tools needed to identify, report, classify, investigate, record, and learn from operational loss events, near misses, and risk incidents. In banks, insurance companies, and financial institutions, operational loss data is a critical input to risk assessment, regulatory reporting, and in some frameworks capital calculation. In government entities, ministries, public authorities, energy, oil and gas, and utility companies, healthcare systems, telecommunications providers, and large corporations, systematic incident and loss management is equally important for preventing recurrence, protecting services, and demonstrating effective risk management to boards, regulators, and auditors. In many organizations, operational incidents and losses are under-reported or recorded inconsistently. Staff may be reluctant to report errors and near misses, definitions and thresholds may be unclear, and incidents may be recorded in different systems by different functions without a consolidated view. Key data such as event dates, loss amounts, recoveries, causes, and affected processes may be missing or inaccurate, and losses recorded in risk systems may not reconcile with financial records. Investigations may focus on symptoms rather than root causes, corrective actions may not be tracked to completion, and loss data may not be analyzed or used to improve risk assessments, controls, and key risk indicators. This course provides a structured and practical approach to operational loss event and risk incident management, informed by international operational risk practices and supervisory guidance and recognized incident investigation methods. Participants learn key definitions, including loss events, near misses, gain events, boundary events, and risk incidents, widely used operational risk event type classifications and how to adapt them for non-financial organizations, and the purposes and uses of loss and incident data. The program covers designing loss data and incident management policies and procedures, defining collection thresholds and data fields, capturing key dates, gross losses, recoveries, and net losses, distinguishing direct and indirect impacts, and assessing non-financial impacts, including reputational, regulatory, customer, and service impacts. Participants then learn how to build a reporting culture, identify and escalate incidents, triage and classify events, coordinate immediate response and containment, conduct investigations and root cause analysis, and develop, track, and verify corrective and preventive actions. The course also addresses linking loss and incident data with risk and control self-assessments, key risk indicators, and scenario analysis, using external loss data, ensuring data quality and reconciliation with financial records, analyzing trends and producing reports and dashboards, regulatory and management notification, confidentiality and speak-up arrangements, supporting systems, and governance roles for an effective loss and incident management framework. The course is delivered through an interactive, application-focused approach with incident classification exercises, loss data recording and calculation workshops, root cause analysis practice, case studies from financial and non-financial organizations, and reporting and analysis exercises. Participants leave with a loss event and incident management policy outline, an event type classification guide, an incident reporting form and data field template, a loss calculation worksheet, an incident escalation and triage matrix, a root cause analysis toolkit, a corrective action tracker, a loss data quality and reconciliation checklist, and a loss and incident reporting dashboard design.

Learning Objectives

  • Analyze the purpose and value of operational loss event and incident management in risk management, governance, and regulatory compliance on Day 1.
  • Apply key definitions, including loss events, near misses, gain events, boundary events, and risk incidents.
  • Apply widely used operational risk event type classifications and adapt them for the participant's organization and sector.
  • Design loss data and incident management policies, procedures, collection thresholds, and data fields.
  • Record key event information, including occurrence, discovery, and recognition dates, gross losses, recoveries, and net losses, and distinguish direct and indirect impacts.
  • Assess non-financial impacts, including reputational, regulatory, customer, service, and safety impacts.
  • Build a reporting culture that encourages timely reporting of incidents and near misses, supported by confidentiality and speak-up arrangements.
  • Apply escalation, triage, classification, and immediate response and containment procedures.
  • Conduct incident investigations and root cause analysis and develop, track, and verify corrective and preventive actions.
  • Link loss and incident data with risk and control self-assessments, key risk indicators, scenario analysis, and external loss data.
  • Ensure loss data quality, completeness, and reconciliation with financial records.
  • Analyze loss and incident trends, produce management and regulatory reports and dashboards, and define governance roles and system requirements by the end of Day 5.

Who Should Attend

This course is designed for operational risk managers and officers, enterprise risk management professionals, internal control and compliance officers, and risk coordinators and champions responsible for collecting, analyzing, or managing loss and incident data. It is also highly relevant for internal auditors, finance and accounting professionals involved in loss reconciliation, fraud and investigation professionals, business continuity and IT incident managers, health, safety, and environmental professionals, and business unit managers responsible for reporting and resolving incidents. It is particularly valuable for professionals in banks, insurance companies, and financial institutions maintaining operational loss databases for risk management and regulatory purposes; in ministries, government entities, and public authorities strengthening incident management and internal control; in energy, oil and gas, utilities, healthcare, telecommunications, and transport organizations managing operational and safety incidents; and in large corporations building integrated governance, risk, and compliance programs. Senior managers and audit and risk committee members who oversee incident and loss reporting will also benefit from the course.

Learning Outcomes

  • By the end of the course, participants will be able to:
  • Explain why systematic loss and incident management is essential for effective risk management.
  • Distinguish between loss events, near misses, gain events, and other incident types.
  • Classify events consistently using a recognized event type framework.
  • Establish clear policies, thresholds, and data requirements for loss and incident recording.
  • Record events accurately, including dates, losses, and recoveries.
  • Assess and record non-financial impacts of incidents.
  • Encourage open reporting of incidents and near misses.
  • Escalate and respond to incidents promptly and appropriately.
  • Investigate incidents and identify true root causes.
  • Ensure corrective actions are implemented and effective.
  • Use loss and incident data to improve risk assessments, controls, and indicators.
  • Report loss and incident information clearly to management, boards, and regulators.
