The OHS Reporting, Forms & Documentation Training Course equips health and safety professionals, supervisors, and administrators with the practical skills needed to develop, complete, manage, and use occupational health and safety forms, records, and reports accurately and effectively. In government entities, ministries, public authorities, energy, oil and gas, and utility companies, construction and engineering contractors, manufacturing and industrial facilities, healthcare systems, logistics and transport organizations, and large corporations, reliable OHS documentation is essential for preventing incidents, demonstrating legal compliance, supporting audits and certification, informing management decisions, and learning from experience. In many organizations, OHS documentation is extensive but not always effective. Forms may be poorly designed, duplicated across departments, or completed as a formality, incident and near-miss reports may be incomplete, delayed, or written in vague and subjective language, and important information may be lost between paper forms, spreadsheets, and systems. Incidents may be classified inconsistently, making statistics unreliable, regulatory notification requirements may be missed, and OHS reports to management may present data without clear analysis of trends and actions. Records may also be difficult to retrieve during audits or investigations, retention requirements may be unclear, and confidential health information may not be adequately protected. This course provides a structured and practical approach to OHS reporting, forms, and documentation, informed by international OHS management system standards, recognized international guidance on the recording and notification of occupational accidents and diseases, and common regulatory practices. Participants learn the purpose and structure of OHS documentation, including policies, manuals, procedures, work instructions, forms, and records, and the principles of document control and records management. The program covers designing clear and practical OHS forms and completing essential documentation, including risk assessments, job safety analyses, permits to work, inspection checklists, toolbox talk records, training and competency records, personal protective equipment records, equipment and plant inspection records, chemical registers and safety data sheet management, emergency drill records, contractor safety documentation, and health surveillance records. Participants then learn incident and near-miss reporting, including internal reporting procedures, regulatory notification requirements, incident classification, and writing clear, factual, and objective incident and investigation reports. The course also addresses OHS statistics and performance indicators, including lost time injury frequency, total recordable incident rates, severity rates, and leading and lagging indicators, preparing monthly and annual OHS reports and dashboards for management, data quality, digital HSE systems and mobile reporting, records retention, confidentiality and data protection, and ensuring audit readiness. The course is delivered through an interactive, application-focused approach with form design and completion exercises, report writing workshops, incident classification and statistics calculations, and case studies. Participants leave with an OHS documentation structure and document control checklist, a set of sample OHS form templates, an incident and near-miss report template, an incident classification guide, an incident investigation report template, an OHS statistics calculation worksheet, a monthly OHS report and dashboard template, and a records retention and audit readiness checklist.
