Course Overview

Human error is a significant contributing factor in operational incidents, quality failures, safety events, and service disruptions across many industries. Understanding why errors occur requires more than identifying individual actions; it involves examining the interaction between people, procedures, equipment, workplace conditions, organizational practices, and management systems. A structured approach to human error root cause analysis helps organizations move beyond blame and address the underlying conditions that make errors more likely. This course explores systematic methods for investigating human error, including human factors analysis, root cause analysis, the 5 Whys technique, cause-and-effect diagrams, task analysis, and the evaluation of organizational and operational conditions. Participants will examine different types of human error, including slips, lapses, mistakes, and violations, while learning how workload, fatigue, communication gaps, interface design, procedural weaknesses, and training deficiencies can influence performance. The course also addresses evidence collection, incident reconstruction, interviewing techniques, and the distinction between individual actions and systemic contributing factors. Through practical exercises and incident-based scenarios, participants will develop the skills to conduct structured investigations, identify verified root causes, evaluate corrective actions, and recommend improvements that reduce the likelihood of recurrence. The course emphasizes learning-oriented investigations, fair accountability, stronger operational controls, and the integration of human factors into safety, quality, and continuous improvement systems.

Learning Objectives

  • Explain the principles of human error analysis and its role in incident prevention.
  • Distinguish between slips, lapses, mistakes, and deliberate violations.
  • Identify human, technical, environmental, procedural, and organizational contributing factors.
  • Apply root cause analysis techniques to investigate incidents involving human error.
  • Use the 5 Whys, cause-and-effect diagrams, and structured investigation methods.
  • Collect and evaluate evidence while maintaining accuracy, confidentiality, and procedural fairness.
  • Conduct effective incident interviews without leading questions or premature conclusions.
  • Assess the influence of workload, fatigue, communication, competence, and workplace design on human performance.
  • Develop corrective and preventive actions that address underlying system weaknesses.
  • Establish monitoring measures to evaluate the effectiveness of interventions and prevent recurrence.

Who Should Attend

Health, Safety, and Environment (HSE) managers and professionals. Quality assurance and operational excellence specialists. Incident investigation and risk management teams. Operations, production, and maintenance managers. Human resources and organizational development professionals. Reliability engineers and technical supervisors. • Develop clear incident descriptions supported by verified evidence.
  • Identify the human factors and organizational conditions contributing to an event.
  • Apply structured investigation tools to determine underlying causes.
  • Conduct fact-based interviews and evaluate incident records, observations, and available data.
  • Distinguish individual actions from weaknesses in procedures, equipment, training, supervision, and organizational systems.
  • Evaluate corrective actions according to effectiveness, feasibility, and risk reduction.
  • Develop recommendations that strengthen error prevention, detection, and recovery.
  • Prepare investigation reports with clear findings, supporting evidence, and assigned actions.
  • Monitor corrective action implementation and assess whether the risk of recurrence has been reduced.

Course Outline

Day 1

Fundamentals of Human Error and Human Factors

  • Understanding human error and its impact on safety, quality, reliability, and operational performance.
  • Distinguishing slips, lapses, mistakes, and deliberate violations.
  • Exploring human factors, including attention, perception, memory, decision-making, and situational awareness.
  • Examining the influence of fatigue, workload, stress, time pressure, and interruptions.
  • Understanding the relationship between individual performance, task design, and workplace conditions.
  • Moving from blame-focused responses toward learning-oriented and fair investigations.
  • Practical Exercise: Analyse an incident scenario to classify the human error and identify the initial human, task, and environmental contributing factors.
Day 2

Human Error Investigation and Evidence Collection

  • Defining the incident, investigation scope, timeline, and immediate consequences.
  • Collecting physical evidence, documents, system records, witness accounts, and operational data.
  • Reconstructing event sequences and identifying critical decision points.
  • Applying structured interviewing techniques and avoiding leading or judgmental questions.
  • Evaluating evidence reliability, conflicting accounts, and information gaps.
  • Maintaining confidentiality, documentation quality, and procedural fairness.
  • Practical Exercise: Build an incident timeline from a simulated case and identify the evidence required to verify the sequence of events and contributing factors.
Day 3

Root Cause Analysis and Human Performance Assessment

  • Applying the 5 Whys technique to investigate underlying causes.
  • Using cause-and-effect diagrams to organize human, technical, procedural, and organizational factors.
  • Examining task complexity, interface design, procedure usability, and control effectiveness.
  • Assessing competency, training adequacy, supervision, communication, and workload management.
  • Identifying latent organizational conditions that increase the likelihood of human error.
  • Distinguishing root causes from immediate causes, assumptions, and individual blame.
  • Practical Exercise: Conduct a root cause analysis of a simulated human error incident and identify the underlying conditions supported by the available evidence.
Day 4

Corrective Actions and Human Error Prevention

  • Developing corrective and preventive actions linked to verified root causes.
  • Applying the hierarchy of controls to reduce reliance on individual vigilance alone.
  • Improving procedures, equipment interfaces, workplace layout, and task design.
  • Strengthening competency assurance, training, supervision, and communication practices.
  • Applying error-proofing principles, checklists, alarms, and verification controls where appropriate.
  • Evaluating proposed actions for feasibility, effectiveness, unintended consequences, and residual risk.
  • Practical Exercise: Develop a corrective action plan that prioritizes system-level improvements and defines how each action will reduce the likelihood or consequences of human error.
Day 5

Investigation Reporting, Action Verification, and Organizational Learning

  • Preparing structured investigation reports with findings, evidence, and root cause statements.
  • Prioritizing recommendations according to risk, urgency, and expected effectiveness.
  • Assigning action owners, deadlines, resources, and verification requirements.
  • Establishing indicators to monitor corrective action completion and effectiveness.
  • Sharing lessons learned while protecting confidentiality and supporting fair accountability.
  • Integrating human error analysis into incident management, safety systems, quality management, and continuous improvement.
  • Practical Exercise: Prepare a complete human error root cause analysis report with verified causes, prioritized recommendations, assigned responsibilities, and measures for evaluating effectiveness.
