Course Overview

This advanced training course provides a practical framework for the casemix systems that link clinical coding to hospital funding. Unlike general health information and documentation programs, it focuses on ICD-10-AM and ACHI coding governance, Diagnosis Related Group (DRG) grouping logic, cost weights, casemix indices, coding audit, and the budget effects of DRG-based payment. The course examines how the Australian Coding Standards guide code assignment, how AR-DRG grouping uses the principal diagnosis, interventions, and episode complexity, and how cost weights, base rates, and trim points turn a DRG into a payment. Participants will calculate casemix indices, benchmark cost and length of stay, design risk-based coding audits, and assess how activity-based funding reshapes hospital budgets and incentives, including the risk of upcoding. Through grouping exercises, record audits, funding simulations, and case studies, participants will build the capability to govern coding quality and use casemix information in funding decisions.

Learning Objectives

  • Explain casemix classification principles and their role in hospital funding
  • Apply ICD-10-AM, ACHI, and Australian Coding Standards principles to coding governance
  • Analyze AR-DRG grouping logic from principal diagnosis to complexity splits
  • Interpret how diagnoses, interventions, and complications affect DRG assignment
  • Understand how cost weights are derived from patient-level costing data
  • Calculate DRG-based payments using cost weights, base rates, and outlier rules
  • Calculate and interpret casemix indices for hospitals and specialties
  • Design risk-based coding audits with clear sampling methods and error measures
  • Detect coding patterns associated with upcoding, undercoding, and DRG creep
  • Assess how DRG-based funding changes hospital budgets, incentives, and behavior
  • Use casemix data to benchmark efficiency, length of stay, and resource use
  • Develop a coding governance and casemix performance framework
